A complete Bio-Psychosocial Assessment is a critical component in developing comprehensive, quality treatment planning. This moduleassists clinicians by presenting a detailed set of questions and prompts to create the fundamental information necessary togenerate treatment and planning documents. The information gathered in the Bio-Psychosocial Assessment is also used in the creation of reportsrequired by regulatory, managed care and accrediting bodies.
An integral part of this module is the Goal Writing and Client Information Tracking and reporting component. Using the information gathered in the Bio-psychosocial Assessment, staff is prompted to create and prioritize treatment issues, and develop individualized goals and objectives for each client. This aspect of the program also prompts for a child’s strengths and the creation of action steps specific to each objective. Using an AchieveIt© model of goal monitoring and evaluation, clinicians and clients create a measurable set of outcomes that enables the generation of an outcomes report, which graphically depicts the degree of change over the course of treatment. Regular reviews developed by staff, specific to each objective, provide an automated, comprehensive report on the impact of services to the clinical team, child, family and payor.

Staff creates progress notes specific to a relevant objective for every child. Prompts tailored to the type of services performed help staff build substantive comments on the services provided to each client and create the documentation necessary to comply with regulatory requirements. Each progress note may be digitally signed, creating an electronic record. Paradigm’s electronic record/digital signature system has been approved by the Commonwealth of Pennsylvania’s Department of Public Welfare. This highly flexible and individualized system guides clinicians and the treatment team, and enhances focused, high-quality treatment interventions. “Urgent Progress Notes” are automatically emailed to the treatment team. This enhances the level of communication and improves the quality of services provided.

The Clinical Pathways document was created to assist staff in selecting treatment methods specific to an individual client’s diagnoses. Beginning with a section on normal development that reminds staff of the appropriate physical, emotional/interpersonal and cognitive stages of child development, Clinical Pathways presents the diagnoses in simple language. Each diagnostic group typically served in children’s treatment programs contains suggested treatment interventions. This assists staff as they consider the appropriate action steps they can utilize to provide treatment. The suggested language andtreatment ideas presented in Clinical Pathways are easily transferred into the Goal Writing module. Each treatment intervention suggested is also defined in Clinical Pathways, which can increase staff skills and the quality of treatment.1

Children who are placed in out-of-home care frequently have connections to biological family and significant others. Recordkeeping associated with such visitation is often required by regulatory bodies. This software tracks the visitation plan created for each child while monitoring the frequency of visitation and incorporating that information into reports.

This module provides a means of tracking critical incidents that occur over the course of treatment. A range of data that includes critical items required by JCAHO, the Commonwealth of Pennsylvania and federal regulations is gathered. The frequency of those incidents is available through graphs to the treatment team as they utilize the goal writing module. This information also provides data to administrative staff who handle risk management and “environment of care” issues.
Utilizing the Youth Outcomes Questionnaire©2 (YOQ), the software assists the treatment team in monitoring symptom reduction as measured by behavioral dysfunction. The YOQ is a 64-item questionnaire that quantifies the severity of disturbance in children and adolescents ages 4 - 17 years old. The subscales monitor:
- Intrapersonal Distress, Somatic
- Interpersonal Relations
- Social Problems
- Behavioral Dysfunction
- Critical Items
This allows comparison of children served against norms established for children in an outpatient mental health service or in community settings. Through the course of treatment, staff can measure improvement by comparing changes in total scores and sub-scores.
AchieveIt©/Goal Attainment Scaling results are presented graphically, which, over the course of treatment, provides a visual trend line of the impact of services.

Maintaining cash flow is a critical component to any successful human service organization. With the recent changes in federally mandated HIPAA compliant electronic billing, agencies need to accurately bill, monitor and reconcile services provided. Paradigm Software© is an accounts-receivable accounting product for typical services provided under Medical Assistance and other third-party liability purchasers. In addition to an electronic billing system, Paradigm is able to produce UB-04 and CMS-1500 invoices in paper formats. This module is a highly flexible Accounts Receivable option to meet the unique billing expectations of each organization and the services it provides.
